Care coordination - Eating Disorders Program

Each family is assigned to a member of the mental health team, which includes psychologists, psychiatrists, occupational therapists, social workers and mental health nurses. The care coordinator is the contact person for the family, and is responsible for coordinating the psychological care, assisting in transitions between programs, liaison between patient/family and systems involved and developing a collaborative care plan, reviewing progress and responding to current needs.  The care coordinator ensures that all other professionals involved in the care of the family are aware and working towards shared goals.
